Find your PCN and Calculate the 25/26 DES Financial Entitlements​

When you need a quick way to find your PCN code, raw or adjusted list size, or check your network's core payments - look no further!

​

How It Works

Our calculator provides an estimation of your DES (Directly Enhanced Services) payments by allowing you to:

​

  1. Select your PCN from the available list

  2. Input your raw list size (as of 1 January 2025)

  3. Enter your adjusted population

  4. Specify the number of care home beds

  5. Calculate the Investment and Impact Fund (IIF) points you expect to achieve

  6. Choose whether you plan to achieve both local and capacity payments

 

For accuracy, you will need to enter your weighted list size. (Your commissioner will provide this. If you do not have the updated list, use your 24/25 figures for an approximation.)

When entering your population figures, do not use a comma.  For example type 55000

IMPORTANT

​

The 'Network DES Contract Specification' states that the Additional Roles Reimbursement Sum (ARRS) should be calculated using the "PCN Contractor Weighted Population" rather than the raw list size.

​

What this means:

​

The "PCN Contractor Weighted Population" is different from both the raw list size and the adjusted population.

​

  • It's a calculated formula based on patient demographics, rurality, nursing/residential home status, and other demographic factors.

  • It essentially gives a higher weighting to patients who typically require more care.​

 

While all other calculations in the tool are exact, you will need to provide the exact weighted list size for accuracy.

Your commissioner will be able to provide you with your weighted list size.
